Those people with their heads in the mouths of the giant stuffed animals are the kids who have already died from the nightmare world. The weird guy is a former classmate – he is x-ing out photos from a middle-school yearbook. Aoi just happened to be next on the page.

The funniest part is that the two antagonists are pretty much just bystanders in this episode and are enjoying watching the happenings, even though they aren’t particularly actively involved…

Regarding the “kareshi” line, Kyouichi was talking to Komaki. Apparently Komaki and Aoi are so close that people jokingly refer to them as a couple (Komaki is the “boy” since she’s the more masculine one, she even uses “boku”). When the girls in Komaki’s kyuudou club realize that she’s in a foul mood because she fought with Aoi, they go “ahh, a marital spat”. And, well, apparently she is jealous of Toono…
